ABANDONMENT OF ANIMALS: If the client does not pick up or call within 3 days after the agreed upon pick up date, a certified letter will be sent regarding the animal's abandonment. If the pet is not claimed and fees paid within the specified time set forth in the letter, the client will relinquish all rights to this abandoned animal. The animal inquestion will become the property of Hickory Pet Inn/Hickory Veterinary Hospital.
Kennel Cough Informed Consent: We make every effort to ensure our facility is free of diseases including Canine Infectious Tracheobronchitits (Kennel Cough) and Canine Influenza (CIV). Despite our best efforts, we cannot guarantee that your pet will not be exposed to a strain of "Kennel Cough" or any other respiratory virus that is not covered by the current vaccine.
With this being said, if your pet does present with signs of an upper respiratory infection (a dry hacking cough that ends with a retch and/or nasoocular discharge) please call us as soon as possible. We need to be informed of any and all potential cases in order to safeguard the pets in our facility.
I, the undersigned, have been informed and fully understand that my pet could possibly contract Infectious Tracheobronchitis or Canine Influenza despite being vaccinated. In the event that my pet does become ill, Hickory Veterinary Hospital will evaluate at no charge but any diagnostics, medications and/or treatment will be at my expense.
Flea and Tick preventative is encouraged but not required to board. If your pet has fleas, Capstar (OTC oral preventative) may be provided at your cost.
Vaccines Required: To prevent the spread of infectious diseases and parasites daycare, playcare and boarded animals must be current on all vaccines and free of internal and external parasites. By writing your name below, you agree that in the event you cannot provide written proof of such vaccines, you authorize the doctor to provide vaccines and parasite control as needed for your pet.
