We provide all bedding, bowls, etc., therefore we recommend you not leave any of your pet's personal items at the hospital. Items can be misplaced. This includes leashes and collars which we recommend you keep with you.
AS THE OWNER YOU ARE RESPONSIBLE FOR MAKING SURE ALL OF YOUR PROPERTY IS RETURNED TO YOU AT THE TIME OF DISCHARGE.
Please note: Hickory Veterinary and Specialty Hospital is not responsible for the loss of these items.
Medical Authorization (Please Sign below):
HICKORY VETERINARY AND SPECIALTY HOSPITAL IS AUTHORIZED TO RENDER MEDICAL TREATMENT FOR ILLNESS OR INJURY (I.E. VOMITING, DIARRHEA, EAR INFECTIONS, ETC.) AS DEEMED NECESSARY BY THE DOCTORS IN THE EVENT OF ILLNESS OR INJURY WHILE BOARDING. If not a life-threatening emergency (as deemed by the doctor) a phone call will be given before any treatment is administered. If a client elects this option, they must leave at least 2 contact phone numbers. If a doctor leaves a message and does not receive a call back within 2 hours, the doctor is authorized to perform an exam and medical care as deemed necessary in case of illness or injury. ALL COST ASSOCIATED WITH MEDICAL CARE WILL BE DUE AT THE TIME OF PICK-UP.
If you have any special circumstances or requests, we will try to accommodate you in any way possible. For instance:
- Diet - Our house food is Science Diet (cat and dog). If this is not what you feed at home, the change in diet may cause your pet's stool to be loose. If you feel this may happen with your pet, or your pet is already on a special diet please feel free to bring your own food and we will gladly feed their usual diet.
- Exercise - our kennel staff exercises your pet two times a day, if you feel he/she needs extra exercise while boarding please let us know. We can customize an exercise plan for your companion.
- Medications - if for some reason your pet is on medication for certain conditions, our technicians can medicate them throughout their stay at the kennel.
For medical services (vaccines, check lumps, ears, legs, teeth, etc) the owner must agree and authorize. Exam fee of $85.
ABANDONMENT OF ANIMALS: If the client does not pick up or call within 3 days after the agreed upon pick up date, a certified letter will be sent regarding the animal's abandonment. If the pet is not claimed and fees paid within the specified time set forth in the letter, the client will relinquish all rights to this abandoned animal. The animal in question will become the property of Hickory Veterinary Hospital.
VACCINATIONS REQUIRED TO BOARD: Dogs must be vaccinated for Rabies, Distemper, Parvovirus, Bordetella, and Canine Influenza. Cats must be vaccinated for: Rabies and FVRCP. You must provide proof that your pet is up to date on these vaccines at least 1 week before boarding. If your pet requires vaccinations, we can schedule those to be done at Hickory Veterinary Hospital. An up-to-date wellness exam with our doctors is required to board and will be done at the time of boarding, if not our client, the cost would be $72.
This form expires 1 year from the date signed.
Thank you for choosing Hickory Veterinary Hospital to care for your pet during your absence. We make every effort to ensure our facility is free of diseases including Canine Infectious Tracheobronchitis (Kennel Cough) and Canine Influenza (CIV). Despite our best efforts, we cannot guarantee that your pet will not be exposed to a strain of "Kennel Cough" or any other respiratory virus that is not covered by the current vaccine.
With this being said, if your pet does present with signs of an upper respiratory infection (a dry hacking cough that ends with retch and/or nasoocular discharge) please call us as soon as possible. We need to be informed of any and all potential cases in order to safeguard the pets in our facility.
I, the undersigned, have been informed and fully understand that my pet could possibly contract Infectious Tracheobronchitis or Canine Influenza despite being vaccinated. In the event that my pet does become ill, Hickory Veterinary Hospital will evaluate it at no charge but any diagnostics, medications, and /or treatment will be at my expense.
